WHY ARE WE ASKING FOR YOUR CONSENT AS A STUDENT?
Your confidentiality as a student is of great importance. By law, the Family Education Rights Privacy Act (FERPA) protects the privacy of your education records as a student aged 18 and older. You also have the right to waive this law. If you choose to waive this law, please indicate the person(s) or entities that you choose to give the Disability Support Services office permission to communicate with on your behalf.
Communication as denoted above may include obtaining and/or releasing student’s historical and/or current information regarding assessment, diagnosis, needs, recommendations, treatment, prior services, academic records, performance, or information that may relate to accommodating student’s needs at Oakwood University. This consent form will be valid until revoked by student. A photocopy of the original consent form shall be as valid as the original consent form.
